The New Patient.
On your first visit to our clinic, you will be asked to complete our New Patient Questionnaire. If you are booking your appointment online, you will be sent the questionnaire via email to be filled out and sent back before your appointment. If you prefer, you may choose to print out the forms and complete them on arrival to our office. Your chiropractor will read these forms and consult with you on your health and lifestyle history followed by a thorough examination of your areas of concern.
